Description

Clomiphene citrate is an oral ovulation-inducing agent used in appropriately selected patients with ovulatory dysfunction who desire pregnancy. It is intended to support follicular development and help trigger ovulation under physician supervision after proper diagnostic evaluation.

Indications

Clomiphene citrate is indicated for the treatment of ovulatory dysfunction in women desiring pregnancy. It is commonly used in anovulatory infertility, including patients with polycystic ovary syndrome (PCOS) when ovulation induction is clinically appropriate.

Dosage & Administration

Use only as directed by a licensed healthcare professional. Standard clomiphene therapy typically begins at 50 mg once daily for 5 days, usually started on or about day 5 of the menstrual cycle; if ovulation does not occur, the dose may be increased in a later cycle to 100 mg once daily for 5 days. Increasing dosage beyond 100 mg/day for 5 days is not recommended in the FDA labeling.

Mechanism of Action

Clomiphene citrate acts on estrogen-receptor-containing tissues, including the hypothalamus and pituitary, and initiates endocrine changes that increase pituitary gonadotropin release. This promotes follicular growth, a preovulatory gonadotropin surge, and subsequent ovulation.

Precautions

Clomiphene citrate is contraindicated in pregnancy, liver disease or prior liver dysfunction, abnormal uterine bleeding of undetermined origin, ovarian cysts or enlargement not due to PCOS, uncontrolled thyroid or adrenal dysfunction, pituitary tumor, and hypersensitivity to the drug. Patients should also be monitored for visual symptoms and ovarian hyperstimulation syndrome (OHSS); new visual changes, pelvic pain, abdominal distention, or rapid weight gain require prompt medical evaluation.

Side Effects

Commonly reported adverse effects include hot flashes, abdominal or pelvic discomfort/bloating, nausea and vomiting, breast tenderness, headache, irregular menstrual bleeding or spotting, and visual symptoms. Serious but less common risks include OHSS and prolonged or, in some cases, possibly irreversible visual disturbance.

Storage

Store at 15°C to 30°C (59°F to 86°F). Protect from heat, light, and excessive humidity/moisture, and keep the product in a tightly closed container.
