Description
Telmisartan is an angiotensin II receptor blocker (ARB) used in the management of hypertension. It helps lower blood pressure by blocking the vasoconstrictor and aldosterone-secreting effects of angiotensin II, thereby promoting vascular relaxation and improving blood pressure control.

Indications
Telmisartan is indicated for the treatment of hypertension, to lower blood pressure. In some labeled products, it is also indicated for cardiovascular risk reduction in certain patients who are unable to take angiotensin-converting enzyme (ACE) inhibitors.

Dosage & Administration
Dosage should be individualized according to patient response and clinical need. The usual starting dose is 40 mg once daily, with a dose range of 20 mg to 80 mg once daily. Blood pressure response is dose-related, and 80 mg once daily is the maximum standard labeled strength for monotherapy. Telmisartan may be taken with or without food.

Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in vascular smooth muscle and the adrenal gland. This inhibits angiotensin II-mediated vasoconstriction and aldosterone release, resulting in reduced blood pressure.

Precautions
Telmisartan should not be used during pregnancy because drugs that act on the renin-angiotensin system can cause fetal harm. Use with caution in patients with renal impairment, hepatic impairment, volume depletion, or hyperkalemia risk. Blood pressure, renal function, and serum potassium should be monitored as clinically appropriate.

Side Effects
Common adverse effects may include dizziness, hypotension, back pain, sinus congestion, and fatigue. Less common but clinically important effects may include impaired renal function, hyperkalemia, and symptomatic hypotension, particularly in susceptible patients.

Storage
Store at controlled room temperature, protected from moisture and light, and keep in the original packaging until use when applicable. Keep out of reach of children and follow the manufacturer’s labeled storage instructions.
